You are the physician associate working in the emergency department of a busy city hospital. 

The next patient you see is a 65 year old lady by the name of Emilia Popov. Her "presenting complaint" on the electronic system states "heartburn"

On introducing yourself you realise Mrs Popov speaks limited english. She gestures to her sister who can translate for her.
